Safety audit checklist
A health and safety audit form that marks each requirement as conforms, nonconformity or observation across walkways and exits, first aid, fire equipment and records. Findings are graded with evidence, a corrective action, an owner and a target date, and both sides sign at the closing meeting. Every question is listed below, and you can fill the live form in on this page.

What should a safety audit checklist include?
A safety audit checklist should state the scope and the criteria the site is audited against, then record a result for each requirement with the evidence seen. Each nonconformity needs a corrective action, an owner and a target date. HSE says larger organisations should have formal procedures for auditing and reporting health and safety performance. Close with a meeting so the auditee sees the findings first.

Try the safety audit form live
Answer a few questions and watch it adapt. Your answers are not saved, and any photos you add are deleted within the hour.
This is the real form as your team would use it: questions appear as you answer, scores work themselves out and you can add photos and signatures. No sign-up needed.
Fill it in on a phone or tablet
Your team completes the safety audit on site in the SiteSurvey app on iPhone, iPad and Android phones and tablets. The project name and address fill in for them, it works without signal, and it syncs when the device is back online with photos kept against the question they answer.
Customers and subcontractors do not need the app: send them a link and they can fill it in from any browser.

Every question in the safety audit template
Workplace audit with walkways, first aid, fire equipment and records, findings and corrective actions, closing meeting. 8 sections and 97 questions. Follow-up questions and guidance appear only when an answer calls for them, so the person on site never sees the ones that do not apply.
Details you only enter once
Add this form to a project in SiteSurvey and it fills in what the project already knows, so nobody on site retypes it and every form on the job uses the same names. Anything filled in can still be changed.

Safety audit template questions
What is the difference between a safety audit and a safety inspection?
An inspection looks at conditions on the day. This audit also checks the site against stated criteria and asks to see records, such as the first aid needs assessment, the fire risk assessment review and the actions from the previous audit. HSE describes periodic audits as checking how well management structures and risk controls are working.
How are audit findings graded?
Each finding is a major nonconformity, a minor nonconformity or an observation. A major one also records what was done to make the area safe and who was told. Major and minor findings both need a corrective action, an owner and a target date, and an observation records an improvement where the requirement is already met.
Is a health and safety audit a legal requirement?
Regulation 5 of the Management of Health and Safety at Work Regulations 1999 requires every employer to have arrangements for the planning, organisation, control, monitoring and review of their preventive and protective measures. An employer with five or more employees must record those arrangements. An audit is one way to show that monitoring and review happens.
What does it check on fire exits?
It checks that routes to fire exits are clear, exits are signed and exit doors open easily from inside. In England and Wales, the Fire Safety Order says routes to emergency exits and the exits themselves must be kept clear where that is needed to keep people safe. It also says emergency doors must not be fastened so that they cannot be easily and immediately opened by anyone who needs them in an emergency.
Can an auditor audit their own area?
The form's guidance is that an auditor should not audit their own work. If an internal auditor works in the area being audited, the form asks for that to be recorded and for the report to say how someone else will check the findings.
Can the auditee sign the audit on site?
Yes. The closing section records whether a closing meeting was held, the auditee's response and any disputed findings, then takes the auditor's and the auditee's signatures on the device. In the SiteSurvey app it works offline, with photos attached to each finding.
