Boiler log and inspection checklist
A boiler log sheet and boiler inspection checklist for commercial and industrial steam and hot water boilers, filled in on a phone in the boiler room. Add each plant room and each boiler in it, scan the data plate, then log readings each round: pressure, temperature, water level, stack temperature, fuel, blowdown, low water cutoff test and initials, against the operating limits you enter. The periodic inspection covers burners, controls, relief valves, gauge glass, low water cutoffs, water treatment and combustion readings, refractory, leaks and flue. For a domestic gas boiler service use the boiler service checklist; for the competent person's examination use the pressure system inspection record.
For stationary engineers, boiler operators, facility managers and maintenance contractors running boiler plants in hospitals, schools, offices and factories.
The form works in the US, the UK and elsewhere: choose where the plant is and it asks for the state or city certificate per boiler, or the UK written scheme of examination and the competent person's examination.
- Plant rooms as spaces and boilers as items, each with a data plate scan, status and certificate
- A shift log per boiler with highest and lowest readings worked out against the limits you enter
- Failed low water cutoff tests and abnormal water levels counted and flagged
- 14 periodic inspection checks per boiler, plus water treatment and combustion readings against your contractors' targets
- Statutory inspection dates, actions with priority and owner, and two signatures

What should a boiler log include?
A boiler log is useful when every reading sits next to that boiler's own operating limits, so a reading out of range or a low water cutoff that failed stands out on the day, not at the next inspection. Each round needs a time and initials, and a failed safety device test needs a note saying what was done straight away.
Last updated . Facts checked by the SiteSurvey team on 27 October 2026 against Pressure Systems Safety Regulations 2000, regulation 8 (legislation.gov.uk), Pressure Systems Safety Regulations 2000, regulation 9 (legislation.gov.uk), Pressure Systems Safety Regulations 2000, regulation 11 (legislation.gov.uk), HSE, Pressure Systems Safety Regulations 2000 (PSSR), HSE, Using pressure systems and Minnesota Statutes 2025, section 326B.958 Inspection and registration.
How to keep a boiler log
Set the operating limits once from the boiler's operating instructions, certificate or maker, then log every round.
Set up the log
Enter the plant, the shift and whether this record is a log, a periodic inspection or both. Add each plant room and each boiler, and scan the data plate.
Enter the limits
For each boiler, enter the lowest and highest normal pressure and temperature and the highest stack temperature, with where they come from. The form supplies none of its own.
Log each round
Record the time, pressure, temperature, water level, stack temperature, fuel reading, blowdown and low water cutoff test, with your initials. The form shows the highest and lowest values and flags any limit exceeded or cutoff that failed.
Do the periodic inspection
Check burner and flame, flame safeguard, controls, relief valves, gauge glass, low water cutoffs, gauges, feedwater, refractory, leaks, flue and blowdown, and enter water treatment and combustion readings against your contractors' targets.
Close out
Record the certificate or written scheme position, give each problem an action with an owner and a date, choose the decision and sign with the plant manager.
What makes a boiler log useful
Shift log: readings per round against the limits you enter, and the low water cutoff test
B-2, 09:30: 10 psi, 239 °F, level normal in the glass, stack 390 °F, blowdown done. Low water cutoff test: did not cut out, burner kept firing below the cutoff mark. Burner shut down by hand, B-2 isolated, plant manager told, burner contractor called. B-1 carrying the load.
Fictional example. Record the actual evidence and approved arrangements for your job.
A common recording mistake
Ticking the low water cutoff test as done without watching the burner shut down and the alarm sound, or carrying on with the boiler on line after a failed test.
What happens next
Keep the failed boiler out of service until the device is repaired and retested, record the retest, and give the plant manager the log with the actions and their owners.

Every question in the boiler log template
A boiler room log and inspection for commercial steam and hot water boilers: plant rooms and boilers with data plate scans, a shift log per boiler (pressure, temperature, water level, stack temperature, fuel, blowdown, low water cutoff test, initials) against operating limits you enter, periodic inspection checks, water treatment and combustion readings against your contractors' targets, the US certificate or UK PSSR written scheme and examination, actions and sign-off. 6 sections and 161 questions. Follow-up questions and guidance appear only when an answer calls for them, so the person on site never sees the ones that do not apply.
Make it your own: every question is editable
This is a starting point, not a fixed form. Once it is in your SiteSurvey account you can reword any question, add or remove questions, sections and answer options, and add your own logo, so it fits the way your team works and the jobs you do.

Boiler log FAQs
Who has to inspect a commercial boiler in the US?
It depends on the state or city. As one example, Minnesota Statutes 326B.958 requires the owner to have a boiler inspected at least annually after the initial inspection; the inspector's label on the boiler, showing the month and year and the inspection agency, is the inspection certificate, and a boiler found unsafe must not run until the defects are corrected and verified. The form records each boiler's certificate number and expiry and the last inspection. Check the rules where your plant is.
Source: Minnesota Statutes 2025, section 326B.958 Inspection and registration.
Is a boiler covered by the Pressure Systems Safety Regulations?
HSE says PSSR covers systems containing a relevant fluid, which includes steam at any pressure and pressurised hot water above 110 °C, and names a steam boiler as an example of an installed system. Under regulation 8 the user must not operate the system without a written scheme for periodic examination by a competent person of the protective devices, pressure vessels and pipework where a defect may give rise to danger. The form asks whether the scheme is in place.
Sources: HSE, Pressure Systems Safety Regulations 2000 (PSSR) and Pressure Systems Safety Regulations 2000, regulation 8 (legislation.gov.uk).
How quickly must the competent person send the examination report?
Regulation 9 of PSSR says the competent person makes a written report of the examination, signs or names it, dates it and sends it to the user within 28 days of completing the examination, or before the date given in the report if that is sooner. The form records the last examination, the next due date and whether the report is held; record the examination itself on the pressure system examination record.
Source: Pressure Systems Safety Regulations 2000, regulation 9 (legislation.gov.uk).
Does the form tell me the safe pressure, relief valve setting or low water cutoff test interval?
No. Those come from the boiler maker, the boiler's certificate, the design and your site procedure. HSE says the user should know the safe operating limits and keep protective devices such as safety valves adjusted to their correct settings and in good working order. You enter the limits for each boiler and the form compares every reading against them.
Source: HSE, Using pressure systems.
Do operating instructions have to be kept at the boiler?
In Great Britain, PSSR regulation 11 requires the user to provide adequate and suitable instructions for safe operation and for the action to take in an emergency, and to make sure the system is only operated in line with them. The plant room checks include whether the operating instructions and emergency procedures are available at the plant.
Source: Pressure Systems Safety Regulations 2000, regulation 11 (legislation.gov.uk).
Is this a boiler inspection certificate?
No. It is an operator's log and inspection record kept with photos, data plate scans and a PDF report you can share. It does not replace the state or insurer inspection, the competent person's examination under a written scheme, or any certificate. It shows what was logged and checked, and what was done about it.
