An installation sign-off for signs. Record the permissions and consents and the access equipment, then add each sign: whether it went in as surveyed, the fixings, height, level and security, lighting tests and who connected it, removal of the old sign, after photos and any snag to return to. The form counts the snags, and the customer signs on the day. Every question is listed below, and you can fill the live form in on this page.
✎Every question is editable. Add this form to your account, then change the wording, add or remove questions and options, and make it fit your own jobs.

What should be checked when a sign is installed?
Check that each sign matches the approved survey and drawing, the fixings suit the wall and the sign's weight, it is level, plumb and secure at the right height, illuminated signs are connected and tested, the old sign is removed and the wall made good, and the site is left clean. Take after photos and get the customer's sign-off.
Record each sign as its own row: where it went, the fixings, the height to its bottom edge, the lighting test and any snag. The form counts the snags to come back for.
Height to the bottom of the sign
Photograph the shopfront with a door or a person below the sign and measure the height to its bottom edge from the photo. An illuminated sign needs to be at least 2.5 m up for deemed consent, and the form says so. It works on web forms in any phone browser today, and the app version is on its way.
Two signs fitted at a fictional coffee shop: a lit fascia sign at 2,650 mm, tested by an electrician, and a projecting sign at 2,600 mm, moved 150 mm to clear a downpipe bracket and left with one snag.
A signage installation sign-off: each sign installed against the survey with its fixings, height, level, illumination and photos, removal and making good, consents and the customer's sign-off. 3 sections and 31 questions. Follow-up questions and guidance appear only when an answer calls for them, so the person on site never sees the ones that do not apply.

Make it your own: every question is editable
This is a starting point, not a fixed form. Once it is in your SiteSurvey account you can reword any question, add or remove questions, sections and answer options, and add your own logo, so it fits the way your team works and the jobs you do.
Details you only enter once
Add this form to a project in SiteSurvey and it fills in what the project already knows, so nobody on site retypes it and every form on the job uses the same names. Anything filled in can still be changed.

CheckThe electrical connection of an illuminated sign must be certified by the electrician to BS 7671: an Electrical Installation Certificate for a new circuit, or a Minor Electrical Installation Works Certificate for work on an existing circuit. This sign-off does not replace that certificate.

A completed sign installation report, made by SiteSurvey from a fictional site visit: every answer, the photos, the guidance that applied and the sign-off, ready to send as a PDF.
Answer that it was not installed as surveyed and record what changed, for example moved to clear a downpipe and agreed with the client on site. The record protects you if questions come up later.
Do I need consent for a new sign?
The GOV.UK guide for England says every outdoor sign needs the permission of the owner of the site, including the highway authority if it is on highway land. Some signs also need advertisement consent from the planning authority. The form records the permissions and consents in place.
What about illuminated signs?
Deemed consent for illuminated signs has limits: for example the sign must be at least 2.5 metres up at its lowest point, and it does not apply in conservation areas. The form records the height, that the lighting was tested and who connected it.
Can we work from a ladder?
HSE says ladders can be a sensible option for low-risk, short-duration tasks, but they should not automatically be your first choice. The form records the access equipment used.
How are snags handled?
Mark a sign as a snag to return to with what needs doing. The form counts the snags, so the return visit is clear.
Does it follow on from a sign survey?
Yes. Use the signage survey template to measure and photograph the elevation first, then this form for the installation, with the survey reference on it.
