TMV servicing and testing record
A service and test record for thermostatic mixing valves (TMVs). Record the thermometer used, then add each valve: its location and outlet, scan its tag, the valve type, set and measured mixed temperatures, the fail-safe test with the cold supply isolated, and whether strainers were cleaned. The form warns when the mixed temperature is above 44°C, records the action and the temperature after adjustment, counts the valves that pass and fail, and ends with actions and sign-off. Every question is listed below, and you can fill the live form in on this page.

What does a TMV service involve?
A TMV service checks each valve's mixed water temperature against its set point with a thermometer, tests the fail-safe with the cold supply isolated, cleans the strainers and check valves, and records the result and any adjustment. HSE says TMVs need regular routine maintenance by competent people in line with the manufacturer's instructions, and that strainers or filters should be inspected, cleaned, descaled and disinfected annually or as the risk assessment says.

Every question in the TMV servicing template
A thermostatic mixing valve (TMV) service record: each valve with its location, outlet and type, set and measured mixed temperatures, fail-safe test, strainers and result, with a warning above 44°C. 3 sections and 56 questions. Follow-up questions and guidance appear only when an answer calls for them, so the person on site never sees the ones that do not apply.

- GuidanceHSE: where vulnerable people are at risk from scalding during whole-body immersion, water temperatures must not exceed 44°C. Where hot water is stored and distributed hot to control legionella, baths and showers should be fitted with TMVs, and outlet temperatures and TMVs should be monitored and maintained so the controls stay effective. HSE says TMVs need regular routine maintenance by competent people, in line with the manufacturer's instructions.
- GuidanceSources: HSE HSG274 Part 2 (legionella control in hot and cold water systems) on TMVs and outlet temperatures, the NHS Model Engineering Specification D08 (thermostatic mixing valves, healthcare premises) and the BuildCert TMV2 (domestic, BS EN 1111 and BS EN 1287) and TMV3 (healthcare) approval schemes. D08 sets maximum mixed temperatures for each outlet type that are lower than 44°C for showers, basins and bidets: enter the figure for each valve from D08 or the site water safety plan. D08 asks for in-service checks a few weeks and again a few months after commissioning, then at an interval set from the results.

TMV servicing FAQs
What temperature is safe?
Where vulnerable people are at risk from scalding during whole-body immersion, HSE says water temperatures must not exceed 44°C. The form warns above 44°C.
What is the fail-safe test?
HSE says type 3 TMVs are pre-set and fail-safe, and should be checked regularly to make sure they fail safe if the cold water supply pressure is interrupted. The form records the test, with the cold supply isolated, for each valve.
TMV2 or TMV3?
HSE's HSG274 says type 2 TMVs, which users can override, are required by building regulations where the scalding risk is low. Pre-set type 3 TMVs should be fitted where the risk is significant and are required at healthcare premises. The form records the type of each valve.
How does it fit with legionella control?
Where hot water is stored above 60°C and distributed above 50°C to control legionella, HSE says baths and showers should be fitted with TMVs so water temperatures do not exceed 44°C. Keep this record with your legionella temperature checks.
Can I scan each valve?
Yes. Tag each valve and scan it on the SiteSurvey app, so readings stay with the right valve visit to visit.
Who uses this?
Water hygiene engineers, plumbers, care homes, schools, hospitals and facilities teams.
