Shower head descaling record
A record of quarterly shower head and hose descaling for legionella control. Record the products used, then each shower or spray tap: scan its tag, the scale or biofilm found, whether it was dismantled, cleaned and descaled or replaced, whether a TMV is fitted, the temperature after one minute and the result, with before and after photos. The form warns when an outlet without a TMV is below 50 °C, counts the outlets done and those with heavy scale, and records the next clean. Every question is listed below, and you can fill the live form in on this page.

How often should shower heads be descaled for legionella?
HSE's HSG274 Part 2 says to dismantle, clean and descale removable parts, heads, inserts and hoses of showers and spray taps quarterly, or as indicated by the rate of fouling or other risk factors, for example areas with high-risk patients. HSE lists scale and biofilms among the deposits that support legionella growth, and people catch Legionnaires' disease by breathing in small droplets of water.

Every question in the shower head descaling template
A shower head and hose descaling record for legionella control: each outlet with its location, condition, descale and disinfection, temperatures and result, against the HSG274 quarterly frequency. 3 sections and 30 questions. Follow-up questions and guidance appear only when an answer calls for them, so the person on site never sees the ones that do not apply.

Shower head descaling FAQs
Why do shower heads matter for legionella?
HSE says people catch Legionnaires' disease by inhaling small water droplets containing the bacteria, and that deposits such as scale and biofilm can support bacterial growth. Showers make droplets, and scale builds up in heads and hoses.
What if an outlet has heavy scale?
HSG274 allows cleaning more often than quarterly where the rate of fouling indicates it. The form counts outlets with heavy scale so you can adjust the schedule.
What about little-used showers?
HSG274 says weekly flushing of outlets that are not in regular use can significantly reduce the risk. Use the legionella flushing record for those.
Why record the temperature?
HSG274 says hot water should reach outlets at 50 °C (55 °C in healthcare premises) within one minute. The form warns when an outlet without a thermostatic mixing valve is below 50 °C.
Can I scan each outlet?
Yes. Tag each shower and scan it on the SiteSurvey app so the record matches the outlet. If a room cannot be accessed, record it as no access with a note.
Who uses this?
Water hygiene contractors, care homes, hospitals, leisure centres, hotels and facilities teams.
